Waiver
All members must agree to the following waiver. Your application will be unaccepted otherwise. If you are under age 18, you must have parental consent.

By becoming a new member or renewing my membership, I hereby agree to abide by the USTA Rules, the GLTA Rules, the IndyTennis Constitution & Bylaws and the decisions of the IndyTennis Board of Directors and its committees. I agree to conduct myself in a sportsmanlike manner. I agree that if my conduct deviates from the USTA Rules, GLTA Rules, the IndyTennis Constitution & Bylaws and/or the decisions of the IndyTennis Board of Directors and its committees, the IndyTennis Board of Directors has the right to suspend or revoke my membership, with a pro rata refund of any fees paid. If any of the above listed rules are in conflict, the IndyTennis Board of Directors shall resolve the conflict in the best interest of the entire league and its resolution shall be the binding decision. I agree that any decision made by the IndyTennis Board of Directors regarding my membership status shall be final, and that I will not hold IndyTennis, IndyTennis members, the IndyTennis Board of Directors, or it individual officers liable in this regard.
Further, I agree that acceptance of my entry for ladder play, tournament play or participation in any IndyTennis event is without assumption of responsibility of any kind by IndyTennis or by its directors and/or designees, or by any of the facilities, public or private, at which such functions are held. I agree to waive any and all rights and claims for bodily and emotional injury, damage to my reputation, or any other harms that I might suffer. I also waive any and all rights to claim from IndyTennis or any of the above-mentioned organizations or individuals, damages which may arise as a result of references to me, by inclusion or exclusion, and/or photographs of me as such references or photographs may appear on the IndyTennis website or any other publication or media coverage of events held by IndyTennis.
